Nursing home residents are at increased risk of adverse drug reactions, falls, hospitalization, and mortality due to polypharmacy. It can be a challenge, however, to know when, how, and which medications to discontinue. In order to help guide clinicians with the deprescribing process, the authors of an article published in Annals of Long-Term Care provide a tip sheet that includes a detailed step-by-step guide on how to safely deprescribe, as well as accompanying tables to help clinicians identify medications that may be potentially harmful or contribute to polypharmacy.
