The hospital-to-hospice handoff is a unique opportunity to teach patient safety in a palliative medicine context. Published in the Journal of Pain and Symptom management, through interviews with palliative care clinicians, a comprehensive curriculum to teach and assess appropriate patient hand-offs during the hospital to hospice transition was developed for palliative care fellows.
