“Demystify Prognosis, Medication Relatedness and Coverage” is a case-based seminar focused on determining prognosis, relatedness, coverage and deprescribing. Tools utilized in this seminar were developed in a noteworthy collaboration of NHPCO Pharmacist Steering Committee members, representing several hospice pharmacy providers and hospice organizations.

Join this interactive seminar to discuss options for handling challenging cases and effectively communicating proposed changes or recommendations to patients, families and other prescribers. Learn how to incorporate clinical assessment with relatedness tools to develop a prognosis-driven determination process.

At the completion of this session, participants will be able to:

  • Define the terms prognosis, relatedness and hospice-covered
  • Apply regulatory algorithms to improve your practice
  • Develop a prognosis-driven process for determining and documenting diagnosis relatedness
  • Apply a patient-centered, symptom-focused process for determining medication coverage and having difficult conversations surrounding deprescribing
