Recognizing the Vital Role of Hospice and Palliative Care Pharmacists and Pharmacy Technicians During National Pharmacy Month

October is National Pharmacy Month, a celebration of pharmacists’ integral contribution to healthcare. Aligned with the focus of the Enclara Pharmacist team, this year’s theme for the month is “Pharmacists: Easy to Reach, Ready to Help.”

True to the theme, pharmacists are now recognized nationwide as valuable members of the interdisciplinary palliative care team and for the first time are included as recommended members of this team in the 2018 update of the National Consensus Project’s, “Clinical Practice Guidelines for Quality Palliative Care”.

Pharmacists play a unique role in the care for a patient at the end-of-life often addressing complex pain or multiple medication needs. The integration of pharmacy expertise with palliative care teams builds assurance in medication decisions amongst the interdisciplinary team, to ultimately lead to a better death. Our specialized team of hospice and palliative care pharmacists and technicians are available 24/7 and work collaboratively throughout all organizational levels to provide guidance and solutions to our clients.

Pharmacy Month gives us an ideal timeframe to thank our pharmacist and technician teams for their impact on care quality for patients at the end-of-life.

Pharmacists and Pharmacy Technicians

We are sincerely grateful to our Patient Care Operations pharmacists and pharmacy technicians who go above and beyond the call of duty to provide the best possible customer service to our hospice nurses partners. Their expertise is essential in developing clinical resources, providing consultations and overseeing fulfillment operations. The duties and responsibilities of our team do not stop there – we credit each of our team’s contributions to helping improve medication use and advance patient care.

ePrescription and Controlled Substances Team

As more hospices, prescribers and states adopt ePrescribingour team efficiently and accurately processes all ePrescriptions, as well as fax copies for controlled substances.  They are focused, flexible and detail-oriented, ensuring that every prescription processed reaches the pharmacist for timely review and dispensing. Their exceptional work is commendable and valued both in and outside of the organization.

Call Center Team

A special “thank you” to our Call Center team of certified pharmacy technicians and pharmacists. Their collective years of experience in hospice and palliative care are evident with the passion and determination they bring to each situation.

Each Enclara team member plays an important role, carefully reviewing the needs of patients to ensure quality care by providing clinical insights, processing prescriptions and delivering consistent service.

Training and Quality Team

Our Training and Quality teams collaborate with our Clinical Education team to develop clinical resources, case studies and support materials for the Enclara Learning Academy, Palliative Pearls Newsletter and presentations at national and state associations. They ensure that internal and external operating procedures are sustainable, efficient and effective, while always remaining focused on continual quality improvement and enhancement of skills.

Clinical Management Team

Finally, our Clinical Management teams of dedicated pharmacists work with each Enclara hospice partner to create a strategic plan to enhance patient care and improve financial outcomes. Each of Enclara’s clinical managers conducts utilization management reviews to lead quality improvement projects and create educational programs specific to the needs of each individual hospice. In fact, our Clinical Management teams are often recognized by our clients for their level of support and dedication for improving end-of-life care.
